The Part You’ll Play This position supports parents and their children, preventing out-of-home placement or current placement disruption. Depending on assigned role, utilizes the Strengthening Families Protective Factors Framework and various evidence-based practices to help families navigate resources to become healthy, stable, and safe. This role offers growth, connection, and team-building opportunities and promotes a culture of purpose and belonging.

• Meet and provide direct services to clients: build relationships, advocate appropriately, communicate clearly, and provide information and education. Coordinate, identify, and facilitate needed services or referrals to reduce or prevent confirmed child abuse or neglect, prevent out-of-home placement or disruption, encourage participation in education or job skills training, and increase family, client and youth self-sufficiency.
• Complete all necessary documentation for enrolled clients (admission paperwork, Service Plans, Service Plan reviews, monthly summaries). Maintain ongoing records of all case management contacts. Create progress summaries and share with participating agencies as needed.
• Complete discharge paperwork and final discharge summaries within the defined time, and share with participating parties. Create aftercare recommendations, referrals, and contacts.
• Ensure all necessary billing information is provided for accurate billing in a timely fashion.
• Maintain accuracy of each client’s file with all pertinent information.
• Facilitate and/or attend necessary staffing for each client/family (staffing may occur at multiple locations with multiple agencies). Maintain awareness of services continuum and its impact on clients.
• Maintain paperwork and releases that enable clients to participate safely and legally in the program.
• Represent Hilltop as a service provider in court proceedings, as required.
• Prepare timely billing summaries and spreadsheets in accordance with Hilltop procedures.

Two years of college or four years of related work experience, or an equivalent combination of education and experience.

• Certificate from Crisis Prevention Institute (CPI) (preferred)
• Valid Colorado Driver’s License
