This role is responsible for assisting the Assisted Living Nurse with support service and case management responsibilities that ensure the delivery of quality services, which entails communication with staff in all departments, families of residents, and other service providers.

  • Essential Duties And Responsibilities:

    Demonstrate ethical and professional behavior that aligns with the values of Hilltop s Assisted Living Communities (ALCs), including Service 1, how we treat each other, and Service 2, how we treat our residents. Service 1 and Service 2 are demonstrated by setting boundaries, being predictable, consistent, kind, and transparent.
  • Conduct oneself as a leader by taking ownership of the program and providing quality of care; commit to the betterment and growth of ALCs by offering solutions, developing processes, promoting communication with others, supporting communication from staff to management, prioritizing tasks, and actively participating in problem resolution.
  • Provide protective oversight of residents, including assisting residents, maintaining a safe environment, redirecting and assisting disoriented or confused residents, applying and following house rules and honoring resident rights, reporting and assisting residents experiencing difficulties, and identifying and monitoring visitors to the facility.
  • Provide social supervision, including helping with transport and ambulation to events, assisting with communication of and attendance to social events, and assisting with general socialization.
  • Ensure sanitation and safety of residents, including monitoring living and common areas, reporting safety hazards, maintaining safety supplies, redirecting residents away from unsafe areas or situations.
  • Maintain knowledge of the facility s policies and procedures, admission & discharge process, and Level of Care criteria.
  • Maintain professional communication with residents, families, providers, and coworkers; act as a liaison with resident families and follow up and document as needed.
  • Review daily documentation (shift report book, anecdotal notes, fall reports, etc.) for tracking, quality assurance, and charting.
  • Implement resident care plan changes per facility protocols in conjunction with the Nurse.
  • Under supervision, conduct pre-admission assessments. Prepare long-term care insurance reports and resident charts.
  • Complete routine reassessments and audits, such as safety checklists and monthly fee audits.
  • Coordinate move-in processes.
  • Facilitate and participate as a member of the routine Multi-Disciplinary Team (MDT) process and ensure updated communication and compliance with the process. Facilitate and coordinate interim MDTs, as needed.

Education & Experience

Associates degree in a healthcare related field and two years of healthcare-related experience, or a combination of education and experience that meets the minimum requirements of the job. Minimum 18 years of age required.
