At our Grand Island VA Community Living Center, nurses are at the heart of everything we do. We are seeking dedicated RNs who are passionate about delivering high-quality, evidence-based care in a collaborative environment.
Whether you're administering treatments, developing individualized care plans, mentoring support staff, or guiding patients and families through safe transitions, your work will directly impact the health, dignity, and quality of life of those who served our nation.
This CLC has a total of 36 beds, providing services to Veterans needing 30-90 days of skilled care, rehabilitation, and/or dementia/ neurocognitive support. The mission of a Community Living Center is to restore each Veteran to his or her highest level of well-being, to prevent declines in health, and to provide comfort at the end of life.
Major duties include, but are not limited to:
* Conduct holistic biopsychosocial assessments to identify patient needs and deliver personalized, age-appropriate care.
* Develop, implement, and evaluate individualized care plans in partnership with patients, families, and the interdisciplinary team.
* Safely administer medications and perform procedures per policy, carefully monitoring effectiveness and patient response.
* Educate and empower patients and families with strategies for self-management, independence, and improved quality of life.
* Coordinate seamless care transitions and discharges that support safety and continuity of care.
* Provide leadership and mentorship to licensed vocational nurses and nursing assistants, fostering collaboration and professional growth.
* Partner with the MDS Coordinator to ensure accurate documentation and proper reflection of resident needs and RUG scores.
* Support staff development through orientation, ongoing training, and skill-building to strengthen clinical excellence.
* Contribute to quality improvement projects that enhance patient outcomes, safety, and satisfaction.
* Actively participate in shaping and improving policies, procedures, and practices that advance nursing care in the CLC.
Preferred Experience:
Long-term care and/or rehabilitation experience.

Work Schedule:
Full time Day and Night shifts available. Opportunity to work 72/80 alternate work schedule (work 72 hours, get paid for 80).

EDRP Authorized:
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
