• Completing initial evaluation and re‑evaluating the patient’s nursing needs during each visit.
• Assuming primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
• Completing assessments specific to hospice care, such as identification of home safety risks, suitability of the home environment, availability of support systems, and community resources.
• Providing appropriate support at the time of death and performing bereavement assessment if requested.
• Providing positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel.

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum one (1) year experience as a nurse in a clinical care setting
• Proficiency in nursing skills as defined by accepted nursing standards
• Ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
• Demonstrated skill in nursing practice and ability to cope with illness, disability, terminal patients, family emotional stress, and tolerance of individual lifestyles
• Ability to handle multiple priorities, documentation requirements, and deadlines
• Reliable transportation and valid automobile insurance coverage
• Physical ability to sit, stand, bend, lift, and move intermittently; ability to lift at least 35 lbs and bear the weight of an average adult effectively

• BSN preferred
• Experience as a nurse in a hospice or home health environment preferred
• Familiarity with Home Care Home Base (HCHB)

• Weekend Rotation
• On‑Call Rotation

$70,000—$85,000 USD
