Encompass Health invites experienced case management professionals to lead our Case Management department, ensuring quality care for patients and families while driving operational excellence.

• Assume responsibility for day‑to‑day operations and human resource management of the Case Management department.
• Oversee the interdisciplinary plan of care and discharge planning process, ensuring effectiveness and appropriateness of services.
• Serve as a patient and family advocate, ensuring services meet patient needs.
• Provide guidance and support to case managers and other staff, including training on caseload management and interpreting regulations and policies.
• Review operations to ensure high quality consistent with organizational standards.
• Build relationships with insurance companies, self‑insured employers, case‑management firms, and other healthcare networks.
• Celebrate employee accomplishments and successes.

• Current CCM® or ACMTM certification preferred.
• Must be qualified to independently complete an assessment within scope of practice.
• Active license required if discipline licensure required by state.
• BSN with RN licensure for nursing; bachelor’s degree minimum for other health professionals; graduate degree preferred.
• Three years hospital‑based case management experience, including utilization review and discharge planning.
• Willing to work weekdays and/or weekends, evenings and/or night shifts; may work religious or legal holidays on scheduled days/shifts.
