Job Description:

- Provide home health physical therapy services to clients in a post-acute care setting, including those recovering from surgeries and falls.
- Cover a 60-mile radius area, treating 5-7 patients per day.
- Utilize EMR:
Home Care/Home Base for documentation.
- Conduct services independently, with no clinical involvement or manager on shift.
- Orientation will range from 3-5 shifts.
- Adhere to dress code: CSH-colored scrubs or business casual.
- Manage care-setting:
Home Health and Hospice patients.

Note:

No weekend or holiday shifts are required. Ability to self-schedule and block scheduling are not applicable.
