Use independent professional judgment, analytical skills, and the nursing process to provide a full range of delegated, interdependent, and independent nursing services to patients. Within the framework of the Seven Dimensions of Patient Care and Benner's Domains of Nursing Practice, demonstrate clinical competence; compassion and customer service orientation; focus on process and outcomes; and cost‑consciousness when assessing, planning, implementing, and evaluating nursing care provided to patients.
Provide patient care that respects patients' rights, dignity, values, culture, preferences, and expressed needs. Assess patient/family needs for information and education across the continuum, plan and implement patient teaching using a variety of techniques, and evaluate effectiveness of educational interventions. Help to alleviate patient fears and anxiety through skillful professional and interpersonal communication. Utilize a variety of pharmacologic and non-pharmacologic approaches to ensure patient comfort and relief of pain.
Involve patient, family, and significant others in development of the plan of care. Collaborate with patient/family, hospital staff, and community agencies to develop discharge plans that prepare patients for continued care needs. Orient and evaluate self‑performance in terms of competency (C), needs training or review (NT), and standards of performance. Complete assessment for risk factors including fall, skin breakdown, VTE, and aspiration;
initiate appropriate interventions for each domain. Perform medication reconciliation upon admission and when the level of care changes. Initiate problems and outcomes list based on medical diagnosis and patient needs. Monitor, document, report patient responses to interventions, and progress toward outcomes. Document nursing interventions, patient education, and evaluations every shift. Educate patient regarding treatment plan, safety measures, medications, and self‑care.
Work with Case Manager to ensure appropriate referrals iew discharge plans with patients, ensuring follow‑up arrangements and compliance with CHF discharge instruction guidelines. Document patient care per established guidelines. Provide therapeutic regimens, demonstrate basic nursing procedures (dressing change, catheterization, NG tube insertion, suctioning), and implement medical interventions as ordered. Assist with patient care activities, maintain peripheral IV therapy, obtain specimens, monitor vitals, and administer medications.
Prepare patients for surgery, perform procedures, troubleshoot equipment, and monitor patient responses. Respond appropriately to life‑threatening emergencies. Support families in crisis, maintain patient confidentiality, report unresolved complaints, promote a safe environment, and recognize ethical issues. Maintain cost‑effective use of supplies, evaluate new products and procedures, and participate in quality improvement activities. Mentor student nurses, facilitate teaching, and comply with safety and education requirements.
Attain appropriate credentials and certifications as required.
- Licensed Registered Nurse credentialed from the Michigan Board of Nursing obtained within 60 days of hire or transfer.
- Certified BLS Provider (Basic Life Support) credentialed from the American Heart Association within 90 days of hire or transfer, or Certified Instructor.
