Schedule:
Full-Time, Nights, 7pm-7:30am, every 3rd weekend, 72 hours per pay. Sign On Bonus: $10,000.
- Use independent professional judgment, analytical skills, and the nursing process to provide a full range of delegated, interdependent, and independent nursing services to patients.
- Demonstrate clinical competence, compassion and customer service orientation within the Seven Dimensions of Patient Care and Benner's Domains of Nursing Practice; coordinate development of a multidisciplinary plan of care and ensure alignment with clinical practice guidelines, age-specific requirements, and regulatory standards to achieve quality outcomes.
- Provide patient care respecting rights, dignity, culture, preferences, and expressed needs. Assess information needs of patients and families, plan and deliver education using varied methods, and evaluate effectiveness of education. Apply professional and interpersonal communication to alleviate fears and anxiety and address pain using pharmacologic and non-pharmacologic approaches, involving patient, family, and significant others in the plan of care.
- Collaborate with patient/family, hospital staff, and community agencies to develop discharge plans that prepare patients for continued care needs.
- Complete assessments for risk factors (fall, skin breakdown, VTE, aspiration) and initiate appropriate interventions (e.g., SKINN Bundle, Fall Prevention Plan, SCD). Perform medication reconciliation on admission or changes in care level; establish, direct, coordinate, and document the plan of nursing care with patient/family.
- Initiate and document problems and outcomes (POC) based on medical diagnosis and patient needs; monitor, document, and report patient responses and progress toward outcomes.
- Educate patients regarding treatment plans, safety, medications, and self-care; document education activities and coordinate with Case Manager for referrals prior to discharge.
- Review discharge plans/instructions with patients, ensuring follow-up arrangements. Ensure core measures discharge instructions are provided for CHF patients.
- Maintain documentation according to established guidelines; administer therapeutic regimens and perform basic nursing procedures (e.g., dressing changes, catheterization, NG tube insertion, suctioning).
- Initiate and assist with all patient care activities including activities of daily living; ensure comfort, safety, and well-being; prevent/minimize skin breakdown; follow clinical practice guidelines; establish and maintain peripheral IV therapy; obtain and process body fluid specimens and blood samples as needed.
- Administer medications, IVs, and blood products per policies; prepare patients for surgery or invasive procedures; perform or assist with procedures according to standards of care and nursing practice.
- Utilize equipment per manufacturers’ instructions; operate and troubleshoot IV pumps, PCA, epidural, feeding pumps, beds, and related devices.
- Monitor patient responses, review work in progress, and ensure timely completion and documentation of treatments, medications, and tests; communicate with physician and other professionals regarding treatment plans and orders.
- Assess and respond to changes in vital signs, lab results, I&O, and pain; provide pain relief and include pain management in discharge planning.
- Recognize acute changes in respiratory, neurologic, cardiac, and urinary status; respond with direct interventions and notify appropriate teams as needed; use SBAR or equivalent data-sharing tools for communication.
- Respond to life-threatening emergencies, perform CPR, check equipment, and coordinate emergency resources for patients and families.
- Contribute to a safe, respectful work environment; communicate effectively with patients, families, and team members; resolve conflicts and support quality improvement activities.
- Provide input on patient care assignments, supervise and assist team members, and support cross-team collaboration as needed.
- Utilize the 4 Ps technique (patient, pertinent issues, picture, plan) for change-of-shift and transfer communications.
- Participate in teaching, mentoring of student nurses, and orientation activities; engage in ongoing personal performance improvement and complete annual safety and unit-specific education requirements (GLI).
- Assist in evaluating new products, procedures, and nursing practices; support department and unit strategic goals; participate in quality monitoring and improvement activities.
- Licensed Registered Nurse credentialed from the Michigan Board of Nursing; requires entry within 60 days of hire/transfer date.
- Certified BLS Provider (Basic Life Support) from the American Heart Association within 90 days of hire/transfer date; or Certified Instructor.
