• Use independent professional judgment, analytical skills, and the nursing process to provide a full range of delegated, interdependent, and independent nursing services to patients.
• Coordinate development of a multi-disciplinary plan of care in accordance with Nursing Problem Care Sets and/or clinical practice guidelines; ensure appropriate length of stay, use of resources, and achievement of quality outcomes.
• Provide patient care that reflects respect for patient rights, dignity, values, culture, preferences, and expressed needs; assess information needs and provide teaching; involve patient/family in plan of care.
• Collaborate with patient/family, hospital staff, and community agencies to develop discharge plans for continued care needs.
• Complete admission/shift assessments, risk factor assessments (falls, skin breakdown, VTE, aspiration), and initiate appropriate interventions (SKINN Bundle, Fall Prevention, SCD, etc.).
• Document nursing interventions (NIC) and progress toward outcomes (NOC); provide patient education and evaluate effectiveness of educational interventions.
• Review discharge plans/instructions with patient prior to discharge; ensure appropriate follow-up.
• Demonstrate knowledge and skill application of basic nursing procedures (e.g., dressing changes, catheterization, NG tube insertion, suctioning) and administer medically ordered interventions and medications.
• Establish and maintain peripheral IV therapy; obtain specimens as needed; monitor vitals, labs, inputs/outputs, and patient responses; respond to changing patient conditions and emergencies.
• Coordinate with Case Manager to ensure appropriate referrals and discharge planning; participate in quality monitoring and improvement activities.
• Provide teaching to patients about treatments, safety, medications, and self-care; document education activities on care plans and shift assessments.
• Maintain confidentiality and promote a safe environment; communicate effectively with patients, families, and health care team; resolve patient concerns as needed.
• Support and participate in unit goals, quality improvement, and mentoring of student nurses; participate in team-based activities and healthy work environment initiatives.

• Licensed Registered Nurse credentialed from the Michigan Board of Nursing obtained within 2 Months (60 days) of hire date or job transfer date required.
• Basic Life Support (BLS) certification from American Heart Association obtained within 3 Months (90 days) of hire date or job transfer date. Or Certified Instructor.
• Education:
