Crisis Services Nurse (LPN / RN) - Goshen

Shifts:
12‑hour shifts, 8:00 p.m. – 8:00 a.m.

- Assess individuals for admission using a medical stability approach
- Assess physical health using a brief health assessment
- Deliver care for minor physical health challenges
- Collect vitals, medical and substance‑use history
- Treat minor wounds
- Administer medication with orders from the center provider
- Facilitate escalation of care when clinically indicated
- Help develop and refine center policies and procedures
- Document all necessary information in patient charts
- Other duties as assigned
- Current Indiana LPN or RN license
- Two years of nursing experience
