Job Summary:
Wise Medical Staffing is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in Goshen, IN. Responsibilities:
Provide individually tailored care to patients in a variety of settings, including home health and per diem assignments
Conduct patient assessments, develop care plans, and implement interventions as needed
Administer medications, perform wound care, and provide respiratory treatments, as prescribed
Collaborate with interdisciplinary teams to ensure coordinated patient care
Maintain accurate and detailed records of patient care and treatment plans
Provide education and support to patients and their families to promote healthy outcomes
Requirements:
Current LPN licensure in the state of Indiana
1 year of experience as a Licensed Practical Nurse
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Strong attention to detail and organizational skills
Ability to lift and move patients as needed
