At Aegis Healthcare we are expanding our Home Health & Hospice teams, providing compassionate care to patients in their homes and are seeking a dynamic and experienced individual for the role of Home Health and Hospice Licensed Practical Nurse, LPN. Compensation: $27 - $34 per hour DOE
Status: Full‑Time employee
Schedule: M‑F 8:00 AM to 5:00 PM
Location: 7227 E Baseline Rd., Ste 129, Mesa, AZ 85209

Join our Aegis family as a Home Health and Hospice Licensed Practical Nurse where you will play a crucial role ensuring our patients receive exceptional care. Your responsibilities include verifying and executing the plan of care as prescribed by physicians and under the guidance of a Registered Nurse. You will provide essential nursing treatments, promptly address patient pain, assist with activities of daily living, and educate patients and caregivers on self‑care techniques.

You will also handle medication administration, ensure the availability of necessary supplies, maintain accurate patient records, and consistently liaise with supervising RNs or physicians on patient assessments and any changes in conditions. You will actively contribute to team meetings to ensure cohesive patient care coordination.

• Current Licensed Practical Nursing License in the state of practice
• 1+ years of Home Health & Hospice Experience preferred
• Ability to work independently, be self‑directed, and flexible
• Ability to use a full range of motion and lift up to 35 lbs
• Effective verbal & written communication & time management skills
• Valid Driver's License (clean record) & reliable transportation
• Proof of current auto insurance & registration
• Valid AZ Fingerprint Clearance Card (we can help)
• Current CPR Certification (we can help) & Negative TB Test (we can test)
