The Clinical Liaison is responsible for conducting comprehensive assessments of patients referred for inpatient acute rehabilitation services, coordinating patient information, and communicating with referral sources throughout the process.

• Respond to and carry out pre‑admission screenings for patients referred to the rehab program.
• Determine timely decisions on patient appropriateness for inpatient rehab.
• Develop and implement best‑practice patient attraction strategies to promote rehab program features and benefits to patients, families, and the community.
• Coordinate gathering of patient information for timely presentation to the Medical Director.
• Communicate effectively with all referral sources at every stage of the referral process.

• Minimum 2 years of clinical experience, including direct patient care in a rehabilitation setting.
• Bachelor’s degree or higher, as required by licensure.
• Licensed PT, RN, OT, or SLP with AHA BLS certification.
