Base pay in range will be determined by applicant’s skills and experience. Role is also eligible for team based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Job Summary

Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person centered dementia-friendly programs that meet the specific needs of residents within the dementia care community. Aligns with Brookdale’s Resident and Family Engagement philosophy to provide person-centered programs, fostering meaningful engagement and improving residents’ quality of life. Recognizes individual abilities and needs, encouraging independence and honoring residents’ habits, routines, interests and preferences in care and programming.

• Support the implementation of person-centered programs to meet individual needs and interests of residents, including the 7 domains of well-being (purposeful, intellectual, social, spiritual, emotional, physical) in a dementia-friendly environment, including sensory-focused small groups for residents who cannot participate in large groups.
• Assist the Program Manager or Coordinator in reviewing individual resident service plans and completing program documentation as required by Brookdale standards or state requirements.
• Help ensure dementia care programs comply with state, federal, and other regulations and meet Brookdale’s mission, policies, procedures and standards.
• Provide guidance to associates on programming/engagement and quality of life for residents.
• Coordinate and participate in community events, including family socials, marketing/sales events and open houses.
• Communicate positively with residents, families, and professionals regarding the Brookdale dementia care program and its benefits; discuss residents’ needs and successes with families.
• Assist in meeting the department’s quality improvement initiatives, including use of the BEST tool.
• Support planning and scheduling programming events outside the community, which may include driving a community vehicle.
• Assist with the volunteer program, including training and supervising volunteers.
• Performs related personal care, housekeeping and dietary responsibilities as needed to maintain service quality per Clare Bridge Program standards.
