Job Title: Licensed Practical Nurse (LPN/LVN) – Home Health/Hospice
Location: Goldsboro, NC
Shift: 8 hours/day | 40 hours/week
Job Summary:
The Licensed Practical Nurse (LPN/LVN) provides skilled nursing care to patients in their homes in accordance with physician orders and established plans of care. Working under the supervision of a Registered Nurse (RN), the LPN/LVN coordinates care with the interdisciplinary team and supports patients and families while ensuring compliance with State, Federal, and Medicare regulations.
Key Responsibilities:
- Assist the RN in assessing patients’ physical, psychosocial, and environmental needs and report outcomes to the RN Case Manager
- Participate in planning, coordinating, and implementing patient care
- Deliver and document nursing care accurately using electronic medical records (Homecare Homebase)
- Educate patients and families on self-care techniques and evaluate their response to care
- Maintain effective communication with patients, families, and healthcare team members
- Follow infection control and OSHA safety standards
- Assist with admissions, discharges, transfers, and end-of-life care for hospice patients
- Identify unsafe environments and report concerns promptly
- Participate in on-call rotation as needed
- Attend required in-services and performance improvement programs
Required Qualifications:
- Active LPN/LVN license in the practicing state
- Graduate of an accredited Practical or Vocational Nursing program
- Minimum of 2 years LPN/LVN clinical experience (home health or hospice preferred)
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto liability insurance
- Proof of TB vaccination and Hepatitis consent/declination
- Proficiency with electronic charting systems
- Strong professional boundaries and communication skills
- Ability to read, write, and speak fluent English
