Summary: The LPN is responsible for administering skilled nursing care to home health and hospice patients requiring intermittent professional nursing services as outlined on the patient’s individualized plan of care delegated by the RN.

Essential Job Functions
  • Creates positive experiences for internal and external customers, meeting expectations with courtesy, tact, and knowledge of services provided by the agency.
  • Assists office with achieving positive patient outcomes and provides comfort to patient/family to facilitate quality of life and dignity at death:
    • Performs all skills, treatments, and procedures as outlined in the patient’s individualized plan of care, per 3HC policies and the North Carolina Board of Nursing Practice Act.
    • Observes signs and symptoms, reports to the RN, notes reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
    • Reinforces teaching completed by the RN and counsels patients and family regarding nursing care needs and preventive/rehabilitative procedures.
    • Assists the RN or physician in performing specialized procedures and duties.
    • Assists patient family and other health‑care team members in providing continuity of care and meeting emotional needs.
    • Reports outcomes of any unscheduled visits to the RN within 24 hours of service provision.
  • Observes and documents patient physical and mental health and home environment, gathering data per the individualized plan of care and skilled nursing report; determines need for immediate intervention.
  • Documents electronically and downloads patient information in Net Smart throughout the day; updates medication profile; submits all paperwork including time and travel daily.
  • Collaborates with supervisor to meet agency’s clinical needs, rotates weekend, after‑hours, and holiday on‑call (with an RN backup call), and assists with coverage of visits in other offices as assigned.
  • Maintains regular communication with supervisor and follows the chain of command.
  • Improves nursing care skills: attends office staff meetings, patient care conferences, record audits, utilization reviews, quality improvement activities, inter‑disciplinary team conferences (IDG) at least 75% of the time, mandatory in‑services, and satisfies CEU requirements of the State Board of Nursing.
  • Demonstrates an interest in personal and professional growth: attends workshops, seminars, webcasts; reads and researches individually; recommends educational opportunities for staff and encourages participation.
  • Demonstrates willingness to be cost effective with agency resources, monitors waste, and uses supplies and equipment safely.
  • Adheres to 3HC personnel policy and performs other appropriate duties as assigned by supervisor to promote successful operations and future growth of 3HC.
  • Requirements
    • Licensed Practical Nurse, currently licensed to practice nursing in North Carolina.
    • Minimum of two years experience as a nurse in an acute care setting (preferred).
    • Current CPR certification (BCLS).
    • Dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public.
    • Competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient’s individualized plan of care.
    • Valid driver’s license and reliable transportation.
