THE ROLE
We are seeking an experienced Registered Nurse (RN) to serve as a Private Care Manager and Clinical Advocate for a single private‑pay client in the Golden, Colorado area. This is a concierge‑style engagement focused on high‑quality clinical oversight, care coordination, and quality assurance.
The RN will work alongside existing caregiving agencies and will not replace routine caregiver staffing. The RN’s primary role is to provide clinical oversight, care coordination, and quality assurance for caregiver services delivered by Family Tree and Visiting Angels, ensuring that care is delivered consistently, safely, and in alignment with the client’s established care plan. When clinically appropriate, the RN may also provide limited in‑home RN‑level care in connection with acute issues, hospitalizations, or transitions of care.
Routine bedside nursing is not the primary focus of this role.
To act as the client’s clinical agent and advocate, supporting the delivery of high‑quality care and responding to the day‑to‑day management needs of the client’s clinical care. The RN will carry out this role at the direction of the client and, when necessary, the client’s Medical Power of Attorney, and will work with caregiving agencies and providers as appropriate.
KEY RESPONSIBILITIES- Provide ongoing clinical oversight and periodic assessment of the client’s condition, with particular attention to Parkinson’s‑related needs.
- Maintain and update the client’s care plan and respond to day‑to‑day clinical issues as they arise.
- Serve as the primary clinical point of contact with caregiving agencies, setting expectations and monitoring quality, continuity, and safety of care delivery.
- Provide targeted clinical guidance and coaching to caregivers to support effective implementation of the care plan.
- Review caregiver schedules for clinical appropriateness and advocate for adjustments to support continuity and safety.
- Coordinate clinically with physicians, specialists, pharmacies, and other providers, and participate in medical appointments when appropriate.
- Provide limited in‑home RN‑level clinical care as clinically appropriate on an as‑needed basis.
- Keep the client and Medical Power of Attorney informed of material clinical developments and care‑related issues and provide appropriate updates to family members as directed.
This role requires 24/7 availability for clinical issues, including evenings, nights, weekends, and holidays. While routine work hours are flexible, the RN must be responsive to urgent matters and changes in the client’s condition as they arise.
COMPENSATION STRUCTUREThis position is structured as a 1099 independent contractor engagement. The flat monthly fee of $2,000–$3,000 is intended to cover all routine services associated with this role, including clinical oversight, care coordination, caregiver guidance, scheduling oversight, and on‑call availability.
In the event of unusual or episodic circumstances requiring significant additional time (such as hospitalization support, extended acute care involvement, or complex transitions of care), additional compensation may be arranged by mutual agreement.
