$200 per visit + mileage reimbursement

Home Health Care, Inc. is looking for a Registered Nurse to perform admission and start of care visits around the Twin Cities on weekends. If you have a passion for home care, we encourage you to apply!

• Admission/OASIS Visits: Perform admission and start of care OASIS visits throughout the Metro area.
• Client Evaluation: Evaluate and assess clients for home health services and create their plan of care.
• Service Recommendation: Recommend services from other disciplines that have not yet been ordered by the physician.
• Collaboration: Collaborate with clients, families, physicians, and other members of the home care team.

• Licensed as a Registered Nurse in the state of Minnesota.
• Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License.
• Prior home health nursing experience.
• Proficiency with Medicare & OASIS documentation.
• Demonstrated ability to work autonomously.
• Excellent at planning, organizing, and communicating.

• Army: Practical Nursing Specialist (MOS 68C)
• Navy: Hospital Corpsman (HM)
• Air Force: Clinical Nurse (46NX)
• Marines: Hospital Corpsman (HM)
• Coast Guard: Health Services Technician (HS)
