Service Care Solutions are recruiting on behalf of the NHS for a Band 6 Community Psychiatric Nurse (CPN) to join a Dementia / Later Life Team in Gloucestershire.

Contract Details

Working hours:

Monday to Friday, 9:00am – 5:00pm
Pay rate: £26 per hour (Umbrella)
Contract type:
Temporary, 3 months initially, with the possibility of extension

Key Responsibilities
Engage in complex mental health assessments and plan appropriate care in partnership with service users
Undertake specialist assessments of complex nursing needs
Utilise the Care Programme Approach (CPA) in line with Trust policy
Provide specialist care, education, and advice to service users and carers experiencing common, severe, and enduring mental health conditions
Act as Care Co?ordinator when required, including working with adults at risk and following supervision procedures
Develop, implement, and evaluate care plans in collaboration with service users, carers, and other professionals
Ensure all parties involved receive copies of care plans and that accurate records are maintained on Trust IT systems

Essential Requirements
Registered Mental Health Nurse (RMN)

Experience with in Dementia, Older Adults, or Later Life services
Strong knowledge of the Mental Health Act and legal aspects of mental health care
Demonstrable skills in risk assessment and risk management
Knowledge of different approaches to mental health assessment
