GDOC has an exciting new opportunity for a part‑time, enthusiastic and experienced Clinical Pharmacist to work for both Gloucester Health Access Centre and Partners in Health.

The successful candidate will become an integral part of the general practice team, working either at Gloucester Health Access Centre or Pavilion Family Doctors as well as being active within a network of wider community groups and their multi‑disciplinary teams. They will work with existing groups and work collaboratively with health care professionals internally and externally.

The role is pivotal to improving the quality of care for patients and operational efficiencies within Practices and therefore requires motivation and passion to deliver excellent service within general practice.

Interviews will be held week commencing 9th February 2026.

Implementing and operating a safe and effective medicine management system within the Practice, providing advice and support for patients and colleagues.

Job responsibilities

Duties Specific to Role

  • Ensure medication management systems are safe and effective, liaising with GPs and managers as required.
  • Ensure that proper processes are in place for medication review, and undertaking medication review.
  • Undertake structured medicine reviews.
  • Implement and embed a robust repeat prescribing system.
  • Review and implement safe prescribing systems for vulnerable patients, improving compliance.
  • Review and implement safe prescribing systems for patients suffering from drug and alcohol dependencies.
  • Encourage cost‑effective prescribing within the practice.
  • Review all safety alerts ensuring the required actions are taken.
  • Reconcile medicines following patients discharge from hospital or clinic appointments, or any other contact with a third‑party provider.
  • Review medications for newly registered patients.
  • Review clinical reporting in Systm One relevant to medicines management, and ensure Practice is meeting its contractual requirements and CQC standards, and achieving KPIs (e.g. QOF).
  • Design and run clinical reports & audits to support best practice, and implement any identified changes required.
  • Implement medication management policies and processes of the Primary Care Network.
  • Liaise with pharmacy team colleagues in the Primary Care Network, and support the implementation of best practice in the PCN.
  • Liaise with ICB pharmacy team colleagues.
  • Ensure (in liaison with the lead GP(s) and practice manager) that all contractual requirements relating to medication are met.
  • Support and advise pharmacy team colleagues, including trainees.
  • Act as line manager to pharmacy team colleagues, including trainees.
  • Support the professional development of pharmacy team colleagues, including trainees.
