Band 6 Social Worker

We are currently recruiting an experienced Band 6 Social Worker to join a dynamic and supportive community-based NHS service in Gloucestershire. This is an excellent opportunity to work in a well-established multidisciplinary team delivering high-quality, person-centred care across health and social care settings.

As a Band 6 Social Worker, you will provide professional leadership in complex case management, safeguarding, and care coordination. You will play a key role in supporting patients, families and carers to navigate health and social care systems, enabling safe discharges, preventing admissions and promoting independence.

You will work closely with nurses, therapists, doctors, mental health practitioners and community services to ensure holistic and joined-up care.

• Manage a complex caseload of adults with health, mental health and social care needs
• Complete Care Act assessments, reviews and support plans
• Lead on safeguarding adults, risk management and Mental Capacity Act assessments
• Coordinate care packages, placements and community services
• Support hospital discharge and admission avoidance
• Work collaboratively within MDTs across health and social care
• Provide professional advice, guidance and supervision to junior staff
• Maintain accurate, timely and compliant records

• Degree or equivalent qualification in Social Work
• Registration with Social Work England
• Proven experience working at Band 6 or equivalent level
• Strong knowledge of Care Act, Mental Capacity Act and safeguarding
• Experience of working in NHS, community or hospital-based settings
• Excellent communication, assessment and report-writing skills
• Ability to manage complex cases and work under pressure
