Senior Rehabilitation Assistant

Position: Permanent – 2 posts available:
Full time 8.30‑16.30 Monday‑Friday and Part time 8.00‑14.00 4 on 4 off shift pattern.
We are looking for an enthusiastic and motivated person to work with people in their own homes, providing training, development opportunities, regular supervision and support.
Key responsibilities include:
- Work with Occupational Therapists, Physiotherapists and Reablement Coordinators to undertake assessments and provide reviews.
- Support individuals in achieving outcome‑based goals, encouraging independence.
- Maintain quality standards as per the Reablement Coordinators.
- Complete assessments, determine risks, manage them, communicate and record feedback.
- Use IT systems to update written notes and electronic records.
- Travel locally by car – a valid driving licence and vehicle use required.
Additional information:
- Service operates 7 am – 10 pm, 365 days a year. Pay enhancements for unsociable hours as per Agenda for Change.

Qualifications & experience- Valid driving licence and access to a vehicle.
- NVQ in a relevant topic (e.g., health care) or other relevant training.
- Experience working in health or social care environments.
- Experience working with people, families and carers in challenging situations.
- Experience in undertaking assessments/review progress and using professional judgement.
- Experience motivating service users and encouraging independence.
- Demonstrated experience with goal setting.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a Disclosure and Barring Service check.
