About this jobJob Description for Adults and Children Safeguarding Care Coordinator Job Title Adults and Children Safeguarding Care Coordinator Line Manager Practice Manager Partner/Safeguarding Lead Contract Type Permanent Working Hours 20 hours per week Location Working across 4 surgeries Job Summary Job Description Purpose of the role The Adult and Children Safeguarding Care Coordinator plays a central role in supporting safe, effective and consistent safeguarding practice across the practice and PCN.
The post holder will coordinate safeguarding processes for adults and children, ensuring safeguarding lists are accurate, actions are followed up, information is shared appropriately, and governance requirements are met.
The role supports Safeguarding Leads by maintaining oversight of safeguarding activity, coordinating meetings, audits and reporting, and ensuring compliance with local and national safeguarding guidance.
The post holder will work closely with clinical teams, safeguarding leads, social care, and partner agencies to ensure safeguarding concerns are identified, recorded, escalated and reviewed in a timely and robust manner.
This is a non-clinical but essential coordination and governance role within the multidisciplinary team.
Main Responsibilities Maintain accurate and up-to-date safeguarding registers and lists for both adults and children, ensuring regular review and follow-up for safe practice.
Coordinate safeguarding processes relating to:
Children subject to Child Protection Plans Children in Need Looked After Children Children in Care Adults subject to Section 42 safeguarding enquiries or plans
Track and follow up new safeguarding referrals, including MARF, MARAC and MASH referrals, ensuring outcomes and actions are recorded and escalated appropriately.
Monitor safeguarding cases pending action, including awaiting MASH outcomes, case conference reviews or escalation decisions.
Coordinate regular safeguarding meetings, including preparing agendas, triaging cases prior to meetings, and ensuring all relevant internal and external attendees are informed and invited.
Record, disseminate and follow up actions from safeguarding meetings and reviews, ensuring accountability and timely completion.
Ensure safeguarding coding on clinical systems is accurate and aligned with local safeguarding guidance.
Support consistent, high‑quality information sharing in line with local safeguarding protocols and information governance requirements.
Standardise safeguarding reporting processes across the practice and PCN. Support safeguarding audits and assurance work as required.
Ensure safe, accurate record keeping using Systm One, including documentation of safeguarding discussions, meetings and reviews.
Support Safeguarding Leads by ensuring dedicated time is protected for complex case discussion, reflection and escalation.
The Safeguarding Coordinator is responsible for disseminating safeguarding information and delivering training to Safeguarding Administrators when processes are updated.
Key Tasks
- Safeguarding List Management and Oversight Maintain and regularly audit safeguarding lists, reviewing at least monthly (or more frequently when volumes increase). Ensure safeguarding registers reflect current status and risk. Proactively identify cases requiring follow‑up, escalation or review. Flag safeguarding risks, delays or concerns to Safeguarding Leads.
- Safeguarding Case Coordination Receive and log safeguarding concerns raised by clinicians and staff. Track referrals and outcomes from safeguarding teams and partner agencies. Maintain clear safeguarding chronologies and case summaries. Ensure safeguarding actions are followed up and documented.
- Safeguarding Meetings and MDT Coordination Coordinate regular safeguarding meetings. Support triage of cases prior to meetings to prioritise risk and complexity. Ensure appropriate internal and external attendees are informed and prepared. Minute meetings accurately using Systm One and agreed templates. Track and chase actions following meetings.
- Governance, Audit and Reporting Support safeguarding audits and quality assurance work. Ensure reporting is standardised and consistent. Support preparation of safeguarding evidence for CQC inspections and internal reviews. Assist in identifying learning themes and areas for improvement.
- Information Governance and Coding Ensure safeguarding coding is accurate and aligned with local guidance. Support appropriate, lawful information sharing in line with safeguarding thresholds. Maintain strict confidentiality and GDPR compliance at all times.
