Hospice Registered Nurse Case Manager (PRN)

We have an exciting opportunity for a Hospice Registered Nurse (RN) Case Manager to join our team on a PRN basis!

Position Overview

As an RN Case Manager, you will play a vital role in delivering exceptional hospice care by providing comprehensive nursing assessments, implementing individualized care plans, and supporting patients and their families with expert guidance and compassionate care.

Schedule

PRN (as‑needed) 20-24 hours per week

What You’ll Do
  • ✔️
    Conduct comprehensive nursing assessments to identify patient needs and develop personalized care plans.
  • ✔️
    Implement, evaluate, and adjust care plans in collaboration with the Hospice Interdisciplinary Team (IDT).
  • ✔️
    Provide education, supervision, and emotional support to patients, families, and caregivers.
  • ✔️
    Ensure high-quality, compassionate care in accordance with hospice guidelines and best practices.
What We’re Looking For

  • Active RN license in NJ.

  • Minimum of two years of nursing experience
    , preferably in home health or hospice.

  • Valid driver’s license, auto insurance, and reliable transportation for field visits.

  • Ability to pass background checks, drug screening, and motor vehicle record review.

Physical Requirements:

This role requires a combination of sitting, standing, walking, bending, reaching, and occasional lifting/carrying. Driving, documentation, and patient interaction are also key aspects of the job. Contact Human Resources for a full job description.
