Job Description
Position: Hospital Pharmacist
Location: Glenwood Springs, CO
Start Date: 03/02/2026
Duration: 21 Weeks (end date may adjust based on start date)
Schedule: Rotating day/evening shifts, 40 hours per week
Shift Times: 06:30 or 08:00 start (Monday–Friday)
Weekends: Rotating weekends, 07:00–12:00
Holiday Coverage: Required
The Hospital Pharmacist is responsible for providing safe, accurate, and effective pharmaceutical care to hospitalized patients. This role supports medication management, clinical pharmacy services, and interdisciplinary collaboration, with a primary focus on patient safety and therapeutic optimization. The assignment will support a facility during an EMR transition from Meditech to Epic and requires strong Epic experience.
Roles and Responsibilities- Review, interpret, and verify physician medication orders for accuracy and appropriateness
- Accurately enter and maintain patient medication profiles in the electronic medical record
- Prepare and dispense medications, including IV admixtures, chemotherapy agents, and parenteral nutrition
- Monitor drug therapies for contraindications, allergies, interactions, and correct dosing
- Perform pharmacokinetic dosing per hospital protocols
- Review culture and sensitivity data and recommend appropriate antimicrobial therapy
- Issue and document controlled substances in compliance with federal, state, and local regulations
- Detect, document, and report adverse drug reactions
- Support formulary management through therapeutic substitutions and cost-effective medication use
- Provide clinical consultation and drug information to physicians, nurses, patients, and families
- Document all clinical activities and interventions accurately
- Participate in quality improvement initiatives, audits, and medication safety reviews
- Assist with staff education, training, and orientation programs
- Actively participate as a member of the Code Response Team
- Hospital clinical pharmacy operations
- Epic EMR proficiency (required)
- IV admixture and sterile compounding
- Chemotherapy and parenteral nutrition preparation
- Pharmacokinetics and antimicrobial stewardship
- Medication safety and regulatory compliance
- Interdisciplinary communication and collaboration
- Accurate documentation and attention to detail
- Active Colorado Pharmacist (CO PHA) license.
- Graduate of an ACPE-accredited School of Pharmacy
- Minimum of 3 years of hospital pharmacy experience
- AHA - BLS.ACLS
