Community Outpatient Pharmacist

Overview

We are looking for a community pharmacist to join a team dedicated to supporting each other in the day-to-day operations and challenges of today’s community pharmacy. Our mission is to improve patient outcomes on hospital admission with medication reconciliation, on discharge with discharge medication dispensation, and in day-to-day life outside the hospital.

Full Time

Hours for Retail Pharmacy: Monday-Friday 7am-5:30pm;
Saturday 8am-4pm;
Sunday closed

Rotating Holidays and Saturdays

• Staff in the outpatient pharmacy and collaborate with interdisciplinary care teams both inside and outside the hospital to fulfill hospital discharge orders while balancing the needs of the community patients served.
• Staff rotating Medication Reconciliation Pharmacist shifts with other outpatient pharmacy staff pharmacists.
• Be adaptable and ready to collaborate with the inpatient and outpatient pharmacy teams and all area healthcare providers.

• Graduate of an ACPE-accredited School of Pharmacy with a BS or Pharm
  
  D degree.
• Current license to practice in Colorado or must obtain within 60 days of hire.
• Immunization Certification with active BLS card.

• Able to work independently with minimal supervision.
• Able to handle frequent interruptions and adapt to changes in workload and schedule.
• Able to set priorities, make critical decisions, and respond quickly to emergency requests.
• Able to meet the pharmaceutical needs of neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncologic, obstetric, chemically dependent, and rehabilitation patients.

Community pharmacist experience preferred.

• Immunization Certified with active BLS card.
