Responsibilities
  • Practices the Well Be mission to help patients lead healthier, meaningful lives by delivering the most Complete Care a9 to their patient panel.
  • Primarily supports in-home, patient-facing visits to meet care needs in the assigned market.
  • Partners with other clinicians in the Care Community managing the health and wellbeing of patients in home settings.
  • Engages attributed patients into the Well Be program and supports care needs to keep patients engaged.
  • Provides educational resources to facilitate staff, patient, and family decision-making.
  • Performs initial patient screenings/physical examinations, creates assessment/plan, and completes documentation in a timely manner.
  • Addresses urgent care needs and supports transition of care visits as needed.
  • Collaborates with the community team to complete full patient care assessment and plan.
  • Orders, interprets, and analyzes diagnostic tests and determines follow-up needs per state supervisory agreement.
  • Prescribes and manages medications and treatments as appropriate per state supervisory agreement.
  • Updates patient records and verifies accuracy at each appointment.
  • Delivers patient care and completes regular reviews with supervising physician per state supervisory agreement.
  • Maintains high-quality care while adhering to patient needs and documentation standards.
  • Focuses on documentation, coding, HEDIS measures, patient engagement, and managing medical expenses.
  • Serves as a resource for ongoing care information, counseling, and guidance for patients with chronic conditions.
  • Utilizes resources and refers to internal/external resources as appropriate (e.g., Well Be behavioral health protocols).
  • Collaborates with the interdisciplinary team and participates in regular team meetings.
  • Maintains active, unencumbered state licensure(s) and national certification(s).
  • Delivers palliative care when needed and supports end-of-life care discussions with patients and families.
  • Facilitates family meetings with the Well Be community team to address complex dynamics and goals of care.
  • Adheres to professional conduct, documentation standards, and care needs.
  • Other tasks needed to accomplish team objectives/goals.
Qualifications
  • Minimum Master’s Degree in Physician Assistant Studies (ARC-PA) from an accredited program.
  • 3-5 years physician assistant experience required; new grads will not be considered.
  • 2+ years of prior healthcare experience in related roles (e.g., MA, CNA, EMT/Paramedic, RN) required.
  • Active, unencumbered Physician Assistant license in the state of IL; residency required.
  • Current NCCPA board certification (required).
  • BLS certification and active NPI number; CPR certification (required).
  • Active, unencumbered DEA licensure (required).
  • Valid driver’s license, automobile insurance, and access to a vehicle (proof of insurance required).
Skills We Value
  • Experience in home care, primary care, hospice, palliative care, or geriatrics preferred.
  • Ability to conduct in-home visits and experience with telehealth.
  • Proficiency with Microsoft Office (Excel, Word) and G Suite; familiarity with EHR, scheduling software, and Ring Central.
  • Strong teamwork, integrity, ethics, and productivity.
  • Experience diagnosing and treating ongoing, chronic, and urgent conditions of older adults.
  • Experience in panel management, primary care, geriatrics, internal medicine, or palliative care; end-of-life communication.
  • Excellent verbal, written, and interpersonal communication skills.
