Interim Care Coordinator Nurse
POSITION SUMMARYThe purpose of your job position as Care Coordinator Nurse is to provide direct nursing care to residents, including the coordination of care that is safe, timely, effective, efficient, equitable, and resident-centered. You are responsible for updating the resident care plans according to the residents condition in coordination with other members of the nursing team. You are also responsible for the education of residents and families on how to follow the current treatment plan.
You will use your direct care nursing expertise to serve as the resident/family advocate and act as liaison between medical professionals, payers and all other stakeholders involved in medical care coordination and execution of the resident’s patient care plan, including the Administrator, Director of Nursing and other staff.

The Care Coordinator Nurse serves as the resident advocate and plays the vital role of facilitating care coordination between medical professionals and all other stakeholders involved in executing the resident’s patient care plan. The Care Coordinator Nurse will treat each resident fairly, kindly, and show them dignity and respect. Such behaviors include:
- Knocking before entering each resident’s room
- Referring to each resident by their name
- Communicating clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies
- Working cooperatively with all departments and multidisciplinary teams
- Maintaining a positive and calm disposition
- Demonstrating patience, initiative and willingness to assist residents that may be difficult
- Relaying all pertinent information concerning a resident’s condition to a charge nurse when required
- Having a commitment to always doing the right thing
