Hours:
Monday - Friday - 8a-5p

Vacation: 80 hours per year - 1st year prorated
AZ Sick Time: 40 hours per year
Driving position – must be willing and able to drive to patients’ place of residence.
Job Responsibilities- Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs; provide a complete physical assessment and history of current and previous illness(es).
- Provide professional nursing care by utilizing all elements of the nursing process.
- Assess and evaluate patient’s status by writing and initiating a plan of care, regularly re‑evaluating patient and family/caregiver needs, participating in revising the plan of care as necessary, and initiating the plan of care and making necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis and develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions; include the patient and the family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsel the patient and family in meeting nursing and related needs.
- Provide healthcare instructions to the patient as appropriate per assessment and plan.
- Assist the patient with activities of daily living and facilitate the patient’s efforts toward self‑sufficiency and optional comfort care.
- Act as Case Manager when assigned by the Clinical Supervisor and assume responsibility to coordinate patient care for the assigned caseload.
- Bachelor’s degree from an accredited school of nursing.
- One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care; one (1) year of hospice care experience is preferred.
- Current licensure in State and CPR certification.
- Responsible for supervising hospice aides.
- Excellent observation, verbal and written communication skills, problem‑solving skills, and basic math skills.
- Must be able to walk and stand for long periods of time; able to lift, position, and/or transfer patients; able to lift supplies and equipment; capable of reaching, stooping, bending, kneeling, and/or crouching; visual acuity and hearing sufficient to perform required nursing skills.
- Personal car for travel and a valid driver’s license.
- Carry personal auto liability insurance coverage.
