Hours:
Monday – Friday, 8:00 a.m. – 5:00 p.m.
Benefits:
Medical/Dental/Vision/Life
Vacation: 80 hours per year – 1st year prorated.
AZ Sick Time: 40 hours per year.
Responsibilities- Perform personal care as needed after assigned a patient and attending to requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
- Provide patient and family with positive communication techniques within ethical and respectful care, including confidentiality.
- Meet patients’ safety needs and use equipment safely and properly (foot stools, side rails, O₂, etc.).
- Give personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.
- Assist in dressing and undressing patients as assigned.
- Plan and prepare nutritious meals, including shopping, as assigned.
- Assist in feeding the patient as assigned.
- Provide proper care and observe patients’ skin to prevent breakdown over bony prominences.
- Assess and report on patients’ condition and significant changes to the Case Manager. Be aware of caregivers or others living with the patient and interpersonal issues.
- Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
- Provide normal range of motion and transfers/positioning as directed.
- Offer and assist with bedpans and urinals, provide assistance as assigned with light laundry needs, and keep the patient’s living area clean and orderly.
- Adhere to the organization’s documentation and care procedures and standards of personal and professional conduct.
- Participate in quality assessment performance improvement teams and activities.
- Respect the patient and family/caregiver environment and personal needs.
- Be able to lift, position, and/or transfer patients. Be able to load and unload medical supplies into a car and into the patient’s home.
- Be able to bend, twist, reach, and pull during shift and operate a computer during shift.
- Be able to drive during shift and communicate accurately with patients and families.
- Be able to be stationary and sit during shift. Be able to walk throughout the office and patient homes during shift.
- Be able to complete tasks, including learning, remembering, focusing, categorizing, and integrating information for decision making, problem solving, and comprehension.
