Lead Specialist, Population Health-Regional (Full-time in Primary Care Setting)

Core Responsibilities
  • Drive CDR efficiency by reviewing patient charts and identifying relevant medical information in advance of CDR.
  • Partner with the Population Health Director to facilitate the execution of backend care report processes and move toward resolution between CDRs, including note amendments and order placement.
  • Lead the Daily Lookback process to review previous documentation, identify actions required to align with clinical guidelines, and develop a strategy for completion.
  • Support care teams by providing assistance in the review of patient information for new and existing patients under different provider panels, ensuring accurate historical data for care teams.
  • Leverage data analytics and strategic thinking to identify key clinical documentation insights and drive high-impact documentation initiatives.
  • Other duties as assigned.
What We’re Looking For
  • Minimum 6 months experience working in a healthcare setting.
  • Bachelor’s degree required.
  • Deep understanding of coding and billing, EMR navigation, Oak Street Health clinical guidelines, morning huddle responsibilities, and CDR responsibilities.
  • Eagerness to advance the CIS program’s efficiency and effectiveness.
  • Strong accountability and discipline against timelines and deliverables.
  • U.S. Work Authorization.

Pay Range

The Typical Pay Range for This Role Is $43,888.00 - $85,068.00. This range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short‑term incentive program in addition to the base pay range listed above.
