Brinton Manor Nursing and Rehab Center | Long-Term Care

Brinton Manor Nursing and Rehab Center is seeking an experienced Social Service Assistant to join our Long-Term Care team. This role is ideal for someone who is compassionate, organized, and experienced working with the geriatric population in a long-term care setting.

  • Minimum of one (1) year Social Services experience in a long-term care setting (required)

Key Responsibilities
  • Orient residents and their support systems to the nursing center and assist with adjustment to the new environment

  • Complete social histories, MDS assessments, and develop care plans addressing social and emotional needs

  • Participate in resident care conferences with residents, support systems, and interdisciplinary staff

  • Advocate for and promote resident rights

  • Receive, document, and forward resident and support system concerns to appropriate staff for resolution

  • Follow all facility policies, procedures, and regulatory guidelines

  • Perform other related duties as assigned

Education & Qualifications
  • High school diploma or GED required

  • Minimum of one (1) year Social Services experience in a long-term care setting (required)

  • Knowledge of social services practices, regulations, and guidelines governing long-term care

  • Experience working with elderly, ill, and/or disabled populations

  • Strong interpersonal, communication, and organizational skills

  • Ability to work independently and as part of a team

  • Patience, professionalism, and a positive, supportive approach

  • Must be in good physical and mental health

  • Physical examination and/or physician documentation of freedom from communicable disease required upon hire
