Full Time, Monday through Friday

• Provide expert patient assessment, triage, education, and ongoing care coordination for IBD and GI patients.

• Perform and monitor GI-related infusions and injections across the East Community, ensuring adherence to protocols and patient safety standards.

• Collaborate with gastroenterologists, advanced practice providers, pharmacists, nutritionists, and social workers to support comprehensive treatment plans.
• Provide biologics, immunomodulators, including prior authorization support, refills, and patient adherence coaching.

• Educate patients and families on disease processes, therapies, self-management, and post-infusion care; promote health literacy and shared decision-making.

• Document accurately in EPIC EMR, follow evidence-based protocols, and contribute to initiatives aimed at reducing hospital readmissions and improving patient satisfaction.
• Help streamline workflows, support clinic throughput, and contribute to a positive team environment focused on reducing burnout and improving efficiency.

• Graduation from an accredited school of nursing.
• Knowledge of the principles and skills needed for nursing, care, examination, diagnostic and treatment room procedures.
• Knowledge of medications and their effects, patient education, principles to develop a self-care program, instruments and common safety practices.
