Overview
BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs. Hospice is the final stage of BJC's continuum of care and provides care to patients with life-limiting illness in the home or skilled nursing facility.
Our Hospice staff address the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and families through a multidisciplinary team.

Evenings - 12 p.m. – 10:30 p.m., 10-hour shifts. Monday through Thursday. Possible Weekend/on-call/holiday rotation.

Role and Responsibilities- Role
Purpose:
Evaluates the client and furnishes services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques. When assigned as case manager, coordinates all aspects of care related to that patient. - Responsibilities:
- Assess patient preferences and barriers to involvement in care, including values, emotional, spiritual, cultural, and population-specific needs.
- Develop, implement, and document individual plans of care with defined goals in collaboration with the interprofessional team, patient, family or caregiver; proactively plan and communicate the plan of care across the continuum.
- Promote respect, equity and empathy in interactions with diverse populations; provide patient-family-centered care and advocate for continuity of care. Integrate data from the team and apply critical thinking to implement care safely and timely.
- Evaluate changes in patient condition, inform and collaborate with family or caregivers, and update the plan of care in the EMR. Ensure evidence-based nursing care and quality outcomes.
