Duties and Responsibilities Referrals
- Manage all new referrals to initiate the intake process, ensuring comprehensive completion of intake assessment forms.
- Obtain and manage service authorization requests from non-Medicare pay sources, ensuring timely authorization for initial and subsequent treatment periods.
- Verify insurance coverage for all new and ongoing cases, ensuring compliance with applicable policies.
- Supervise the credentialing process with third-party Managed Care Organizations (MCOs) and ensure ongoing credential maintenance.
- Collaborate with the Billing Coordinator to obtain necessary documentation from physicians, support coordinators, and case managers.
- Oversee and maintain the referral portal, proactively engaging in warm calling to convert referrals into new clients.
- Manage the entire process of obtaining service authorizations, including annual renewals, to ensure uninterrupted service delivery.
- Provide direct supervision to an assigned team of Caregivers, ensuring their performance meets quality standards.
- Coordinate and oversee both in-person and virtual "Meet & Greets" between assigned Caregivers and Clients/Families, ensuring compatibility and comfort.
- Maintain regular communication with contract agencies to foster positive relationships and gather necessary information.
- Act as the primary point of contact for all Client/Family needs, promptly addressing concerns and ensuring satisfaction.
- Assist with scheduling to ensure optimal caregiver assignments for each shift, with readiness to recommend reassignments when necessary.
- Collaborate closely with the clinical team to schedule new client assessments and renewals, ensuring continuity of care.
- Monitor and ensure the effective implementation of the client's plan of care, adjusting as needed to meet evolving needs and preferences.
- Perform accurate data entry and maintain oversight of various client platforms to ensure up-to-date and reliable information.
- Create and manage weekly and quarterly reports to track key metrics and performance indicators.
- Participate actively in monthly caregiver meetings to discuss updates, challenges, and improvements in client care.
- Ensure completion of all necessary new client forms, such as consent for treatment and service agreements, either electronically or manually, and securely save completed documents.
- Assist with on-call duties after hours, including weekends (typically two assigned weekends per month), responding to client and caregiver needs promptly and effectively.
- Manage multiple phone lines, handle email communications, and perform faxing/scanning of documents to support efficient office operations and communication.
Job Type: Full-time.
Pay: $20.00 per hour.
Expected hours: 40 per week.
Benefits: PTO.
Schedule: Monday to Friday.
Work setting: In-person.
Education: High school or equivalent (Preferred).
