Overview

The Role

The post holder will deliver high quality, culturally sensitive nursing care to people of all ages in the community. Working under the direction of the District Nurse, they will provide skilled nursing interventions, support caseload management, and contribute to safe, effective, person-centred care.

This 18month fixedterm position is ideal for an experienced Registered Adult Nurse who is motivated to work autonomously within community services. The role involves assessing patient needs, implementing agreed care plans, and supporting the wider district nursing team. Applicants must demonstrate strong communication skills, ongoing professional development.

Duration, Location, and Working Pattern
  • Location based in Milend Centre
  • Fixed-Term Contract (18 months)
  • Part-time position (15 hours per week)
  • The shift pattern for this post is 3 evenings ) over 7 including weekends and public holidays
  • Please note the salary is Pro Rata for part time hours.
Key Responsibilities

The post holder is responsible and accountable for assessing, planning and implementation of programmes of care as directed by district nurse. These will be developed in partnership with Patients, Family, Carers and Significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health.

The post holder may be required to assume the responsibility of the caseload holder during short periods of sickness, absence and annual leave, seeking support as required from other senior members of the district nursing team.

  • The post holder will be responsible for the utilisation of various clinical and environmental risk assessment tools in order to identify actual and potential risks and ensure implementation of appropriate interventions. The post holder will be responsible for reporting and participating in the critical appraisal of incidents to reduce any recurrences and participate in further investigation as required.
  • As a member of the district nursing team the post holder will be able to support patients, families and carers in order to achieve optimum health and independence.
  • Plan and prioritise workload as required, delegate work using available resources and make decisions to ensure that patients’ needs are being met.
  • Work within systems to ensure continuation of a quality 24-hour nursing service
  • The post holder may be required to provide care of patients within a Treatment Room or Health Centre setting
  • The post holder will use a systematic, flexible and innovative approach to problem-solving where there is either barrier to understanding or financial constraints within the patient’s home, in order to provide a safe and effective care package.

  • Part 1 NMC Registration, or 2 Level Registration with General Nursing Experience
  • Evidence of continuing professional development
