What you will do:

• Conduct comprehensive assessments of patients’ needs in various acute wards.
• Develop and implement effective treatment plans tailored to individual patients.
• Facilitate safe and efficient discharge planning in collaboration with community services.
• Support patient flow and manage discharge processes to reduce hospital stay times.
• Work five days over seven, potentially including weekends, to meet the demands of the service.

Requirements:

• HCPC registration
• Minimum of 2 years’ experience in acute hospital settings
• Strong communication and organisational skills
• Car driver preferred to accommodate potential community follow-ups
