Overview
HC-One Scotland is developing a Specialist Dementia Care Community (SDCC) at Darnley Court care home. Darnley Court has provided care for people living with dementia for many years, and following investment in environmental layout and design, workforce and L&D in Carmichael community, we have opportunities for those passionate about supporting people emotionally distressed by dementia in a specialist service.
Role and SettingAs an SDCC, Carmichael will become a therapeutic care setting that values quality of care and quality of life equally. It will provide a bespoke living environment embracing space and calming de-escalation rooms, with enhanced staffing and specialist training for colleagues in positive approaches to behaviour, psychosocial therapies, de-escalation interventions and safe restrictive practices. The therapeutic ambition is that people will be understood as having needs to be met, rather than defined by their condition.
Key ResponsibilitiesThe Dementia Nurse Practitioner will support, deliver, and coordinate person-centred dementia care and therapeutic interventions in a purpose-designed residential setting to improve clinical outcomes for those living with complex dementia presentations, working within a team of nursing, social care, psychology and wellbeing practitioners. An active NMC Pin & Minimum of 2 years practice experience in a dementia care community is essential for this role.
Skills and AbilitiesEssential Skills & Abilities
- Evidence of delivering person-centred care and care for people with dementia who present with complex needs, distress, and behaviours that challenge.
- Evidence of sharing knowledge and delivering best practice.
- Evidence of using critical reflective practice.
- Collaborative and MDT working.
- Evidence of continued professional development
- Experience of providing general nursing and bio-psychosocial interventions.
- Experience of working as an autonomous practitioner.
Desirable:
- Knowledge of relevant current dementia policies and guidance
Essential:
- Post-registration clinical experience of working with people with dementia.
- Experience of completing holistic nursing assessments
- Experience of working as an autonomous practitioner
Desirable:
- Experience of providing positive behaviour support and psychosocial interventions.
- Experience of auditing/evaluating clinical practice
Essential:
- Good communication skills, flexibility, and creativity.
- Ability to operate effectively within a constantly developing relationship with key stakeholders and associated agencies.
