This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. The RN is accountable for the quality of nursing services delivered by self or others under their direction.
Core Functions- Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history, physical, and diagnostics; reports abnormal results to physicians and relevant caregivers; completes assessments and reassessments as outlined in policy.
- Formulates a plan of care, including the discharge plan, based on assessment data and patient/family input. Sets realistic, measurable goals in collaboration with the patient, family, and multidisciplinary team; revises plan as patient needs change.
- Implements the plan of care through direct patient care, coordination, delegation and supervision of the health care team; provides care per physician orders and nursing plan; ensures compliance with policies, procedures, standards of care and regulatory requirements.
- Evaluates patient and family response to care and teaching; revises the plan of care as needed; collaborates with other team members to evaluate progress toward goals and expected outcomes.
- Documents assessment, planning, implementation and evaluation in the patient record; ensures documentation is legible, timely, and in accordance with policy.
- Provides evidence‑based care and may participate in research or unit/shared leadership activities; mentors and precepts when appropriate.
- Contributes to society through activities that lead to excellent patient outcomes and participates in professional organizations.
- May supervise staff and oversee workflow of the department.
- Interacts with all levels of staff, physicians, patients, families and external contacts, including other health care institutions and community providers.
- Valid RN license in state of practice or temporary/interim RN license (compact accepted).
- BLS certification required; additional certification contingent on area of practice.
- At least one year of experience in an acute‑care hospital setting; experience in the specialty area within the past 12 months required.
- Must maintain clinical performance competencies appropriate to the area as demonstrated through annual validations.
- For certain positions: valid driver’s license and auto insurance (Home Health). Arizona/Banner specific fingerprint clearance cards required for behavioral health positions.
- Bachelor’s degree.
- Professional certification(s) preferred.
