Job Type: Exempt, Fulltime

The Case Manager (CM) is responsible for providing care coordination including needs assessment and identification of care options, communication with patients and families in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

• Bachelor s or Master s degree in Social Work, or as required by state regulations (verify state-specific requirements).
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Minimum of 2 years of social work or case management experience in an inpatient setting highly preferred; experience in acute or rehabilitation hospital preferred.
