Registered Nurse Assessment Coordinator (RNAC / MDS RN)
Location: The Gardens at Gettysburg | Gettysburg, PA
Status: Part-Time (Approximately 16–24 hours per week)

The Gardens at Gettysburg, a skilled nursing facility in Gettysburg, PA, is seeking a passionate and detail-oriented Registered Nurse Assessment Coordinator (RNAC/MDS RN) to join our team. This part-time opportunity allows you to make a meaningful impact by ensuring high-quality resident care through accurate assessments, care planning, and regulatory compliance.

As the RN Assessment Coordinator, you will:

• Conduct routine and unscheduled resident assessments related to condition changes, hospital discharges, and clinical events

• Accurately observe, assess, and communicate changes in resident condition to the interdisciplinary team

• Collaborate with Nursing, Social Services, Activities, and Dietary departments to ensure timely completion of MDS assessments and CAAs

• Complete MDS sections, quarterly reviews, CAAs, and care plans in compliance with state and federal regulations

• Develop, update, and monitor individualized resident care plans

• Prepare and distribute the MDS schedule to the interdisciplinary team

• Transmit MDS data to the state weekly (or as required) and maintain accurate, organized records

• Active Registered Nurse (RN) license in the state of Pennsylvania

• Prior experience in long-term care required

• Strong background in MDS coordination and assessment processes

• Knowledge of state and federal MDS and regulatory requirements
