Status: Full-time;
Monday - Friday; 40 hours per week
Shifts: Monday
- Friday- 9:00-5:00
Direct Supervisor: Executive Director or Program Manager
OverviewThe Family Promise of Hall County Family Advocate provides case management services to current and past guests along with day center clients. Family advocate also provides case management services to guests in our Next Steps Affordable Housing Program. This position supports program initiatives through data entry regarding volunteer information, volunteer hours and family dinner coordination.
ResponsibilitiesCase Management
- Case Management: 13 Families shared between two familiy advocates and a program manager
- Develop individualized case management plan for each case
- Provide crisis management, advocacy and referral services to guests as needed
- Ensure compliance of all guests with program guidelines and report violations to the Executive Director
- Provide case management on and as needed basis to clients who come to the day center
- Complete weekly inspections at transitional home per case basis
- Weekly case management meeting (depending on clients schedule)
- Conduct a weekly meeting with family to review case plan, track progress, set goals, deal with any issues, and provide guidance, counseling and resource referrals. Set weekly goals with adults in the following areas as needed:
- a. Employment
- b. Education
- c. Housing
- d. Finance/budgeting/income
- e. Health – Mental and physical (adults, as well as children)
- f. Parenting (positive approach rather than punitive)
- g. Credit
Administrative Duties
- Answer phones as backup to front desk volunteers
- Document case notes in Family Link for each assigned case
- Schedule and conduct intake interviews with fellow case managers
Lunch & Learn
- Assist with lunch and learn food as needed
DESIRABLE

Skills Needed:

- proficient technology knowledge and skill.
- computer (Google docs and Family Link)
Ability To:
- carry out duties with a minimum amount or no supervision
- make mature and responsible decisions
- maintain accurate records
- offer hope and motivate others
- celebrate small successes and be tolerant of families
- work well with a variety of people
DESIRABLE
QUALIFICATIONS:
- Bachelors of Social Work, Human Services or related field or three years experience in a similar position.
- Any work and/or life experiences which contribute to maturity, sensitivity to others and motivation to work in a community action agency is helpful
- Experience in crisis intervention, problem solving, and resource allocation
- Passion for the program and its goals
- Valid driver’s license and a good driving record
