As a Full-Time MDS-RN at Countryside Post Acute, you will engage in a variety of responsibilities to ensure the well‑being of our residents. Each day, you will conduct comprehensive assessments to develop personalized care plans, working closely with interdisciplinary teams to implement effective strategies. Daily documentation and maintaining compliance with state and federal regulations will be essential.
You will regularly communicate with residents, families, and staff to monitor progress and adjust care plans as needed. Part of your role will also include participating in care conferences and collaborating on Quality Improvement initiatives to enhance overall care standards. Additionally, you will stay current with industry best practices, contributing your insights to foster a culture of continuous improvement.
