Reports to: Property Manager/Portfolio Manager for Enriched Housing Services/ Program Manager

PurposeAccording to the prescribed policies and procedures of the organization, including all applicable federal, state, accreditation, and regulations, and under the general supervision of the Property Manager/Staffing Manager/Program Manager, the Service Coordinator assumes responsibility for coordinating programs and services to help participants maintain a good quality of life and age in place. The Service Coordinator prioritizes a person‑centric model of service delivery.
EssentialFunctions and Basic Duties
- 1. Coordinate group programs and activities for eligible individuals.
- Serve as liaison to community agencies, network with providers, and seek new services.
- Identify low‑cost providers and negotiate discounts.
- Monitor ongoing provision of services and document use in the management system.
- Develop and maintain a Resource Directory of state and local service providers.
- Organize educational events on health care, agency support, life skills, and referral sources.
- Identify partners to provide health services and screenings at participants’ homes.
- Assist participants in building informal support networks.
- Engage volunteers per National Church Residences Volunteer Policies.
- 2. Coordinate individual services through a person‑centered, non‑clinical process.
- Outreach to all eligible participants to provide education and enrollment.
- Conduct assessments and screenings via phone or in‑person.
- Collaborate with care and service colleagues to achieve participant goals.
- Provide quality customer service, addressing questions and basic needs.
- Assist participants in acquiring community services to address social determinants of health.
- Adhere to follow‑up and monitoring schedules per participant preference or program requirements.
- 3. Perform related duties as required or assigned.
- Perform miscellaneous tasks, gather reports, attend interdisciplinary meetings.
- Build rapport with participants while maintaining professional boundaries.
- Manage time independently to meet documentation deadlines.
- Respond to communications promptly.
- Accurately record labor time per policy.
- 4. Documentation.
- Complete timely electronic and written documentation.
- Submit all required forms for participants utilizing the program.
- Complete Care Plans and monitoring with participants.
- Submit documentation needed for funder and performance reports.
- Meet requirements outlined in the Quality Assurance Review Process and File Review Process.
- Carry out responsibilities consistently with National Church Residence’s Core Values.
- Maintain caseload ratio up to 1.25 assigned units/individuals per scheduled hour per pay period.
- Demonstrate positive, professional manner; manage time efficiently; provide leadership and teamwork.
- Adhere to all policies, procedures, and legal regulations outlined in the National Church Residences’ Employee Information Guide (EIG).
- Ensure care respectful of individual rights; report complaints and allegations to supervisors.
- Complete all annual education requirements on time.
- Education/Certification: High School Diploma or GED required; bachelor’s degree in social work or related field preferred.
- Experience: Two or more years in social service delivery with elderly, people with disabilities, and/or family populations.
- Skills/Abilities: Good communication, comprehension, and interpersonal skills; working knowledge of Windows, Microsoft Office, and Web; computer literacy test score required.
- Travel: occasional as directed.
- Vision: normal, consistent with standard workflow.
