Registered Nurse – Home Hospital (Hospital in the Home)

South Metropolitan Health Service (SMHS) is looking for an experienced and compassionate Registered Nurse to join our Home Hospital team. The role delivers acute-level care to patients in their own homes, supporting recovery and reducing the need for hospital stays.

• Provide comprehensive, evidence‑based nursing care within scope of practice.
• Promote patient safety and quality of care through multidisciplinary teamwork.
• Deliver care in patients’ homes or, on occasion, at Home Hospital clinics.
• Reduce length of hospital stay and likelihood of emergency hospital care.

• Valid current CV.
• Statement addressing the selection criteria (2–3 pages).
• Names and contact details of two referees.
