Sussex County, DE.

Pay Range: $42,000 - $47,000 per year.

We are seeking a dedicated Re-Entry Case Manager to support justice-involved youth as they transition back into the community.

In this role, you will strengthen family relationships, address underlying needs behind behaviors, and connect youth to resources that promote long-term success.

• Bachelor’s degree in Social Work, Psychology, Human Services, Criminal Justice, or related field OR 10+ years of experience working with justice-involved youth
• Valid driver’s license, auto insurance, and reliable vehicle
• Ability to pass all required background clearances
• Ability to pass pre-employment, post-offer drug screening
  
  Willingness to work non-traditional hours, including evenings and weekends

• Provide case management for youth returning from placement
• Conduct assessments and develop service plans
• Facilitate meetings in homes, schools, and community settings
• Connect youth to education, employment, mentoring, counseling, and life skills resources
• Collaborate with probation officers, courts, and service providers
• Maintain timely documentation and case notes
