• Attractive pay:
  Salary Range $82,000 - $87,000
• Hours:
  
  Monday – Friday, 8am – 5pm; no on-call requirement
• Holiday work rotation: 1 summer and 1 winter holiday
• Territory:
  Position is for growth — float anywhere needed
• Value added: mileage & cell phone reimbursement; benefits start on the first of the month after hire; including 401(k) match

• Make a meaningful impact on patients and families in their homes

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so
• Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness, according to care center policies
• Record pain/symptom management changes and outcomes as appropriate
• Teach patients, families, and caregivers appropriate patient care techniques
• Regularly communicate with clinical manager and care team
• Perform on-call responsibilities and on-call services to patients/families as assigned
• Other duties as assigned

• One year of nursing experience
• Current RN license, specific to the state(s) you are assigned to work
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance
