Salary Range $82,000 - $87,000 - Hours:
Monday – Friday, 8am – 5pm; no on-call requirement - Holiday work rotation: 1 summer and 1 winter holiday

Position is for growth — float anywhere needed - Value added: mileage & cell phone reimbursement; benefits start on the first of the month after hire; including 401(k) match

- Make a meaningful impact on patients and families in their homes
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so
- Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness, according to care center policies
- Record pain/symptom management changes and outcomes as appropriate
- Teach patients, families, and caregivers appropriate patient care techniques
- Regularly communicate with clinical manager and care team
- Perform on-call responsibilities and on-call services to patients/families as assigned
- Other duties as assigned
- One year of nursing experience
- Current RN license, specific to the state(s) you are assigned to work
- Current CPR certification
- Valid driver's license, reliable transportation and liability insurance
