• Conduct comprehensive assessments to develop individualized care plans for hospice patients.
• Administer medications and treatments as prescribed, ensuring patient comfort and safety.
• Provide emotional support and education to patients and their families regarding end-of-life care.
• Collaborate with interdisciplinary teams to coordinate patient care and services.
• Document patient progress and care interventions accurately in electronic health records.
• Participate in on-call rotations to provide after-hours support as needed.
• Monitor and manage symptoms to enhance the quality of life for patients.
• Advocate for patients’ needs and preferences in all aspects of care.

• Valid RN license in the state of Texas with hospice experience preferred.
• Strong clinical assessment skills and knowledge of pain management protocols.
• Excellent communication and interpersonal skills to engage with patients and families.
• Ability to work independently and as part of a collaborative team.
• Compassionate demeanor with a commitment to providing high-quality patient care.
• Basic Life Support (BLS) certification required;
  Advanced Cardiac Life Support (ACLS) preferred.
• Flexibility to work various shifts, including evenings and weekends.
• Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
