Overview

Klarus Home Health is seeking a PRN / Per Diem
Licensed Physical Therapist to provide PT services to patients enrolled in our home health program in the Dallas/Dallas South region and surrounding areas. This position offers an opportunity to make a meaningful impact on patients  lives in a dynamic and supportive environment. Responsibilities
  • Evaluate patients  physical and functional abilities to develop individualized care plans.
  • Plan, implement, and modify physical therapy treatment programs based on medical referrals and assessments.
  • Provide hands-on therapy, exercises, and techniques to improve mobility, relieve pain, and restore function.
  • Educate patients and their families on therapeutic exercises, proper body mechanics, and injury prevention.
  • Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care.
  • Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans.
  • Participate in ongoing professional development to stay current with trends and best practices in physical therapy.
  • Willingness to perform a Saturday rotation approximately once every 4–6 weeks.
Qualifications
  • Graduate of an accredited School of Physical Therapy.
  • Current State license as a Physical Therapist, or a multi-state PT Compact license if practicing in a PT Compact state.
  • Minimum of two years of experience as a Physical Therapist in a clinical care setting;
    Home Health, geriatrics, or medical-surgical experience preferred.
  • Excellent interpersonal and communication skills.
  • Strong nursing skills as defined by generally accepted standards of practice.
  • Reliable transportation with valid auto liability insurance.
  • Ability to work flexible hours and travel locally as needed.
  • Ability to lift up to 50 lbs. and perform physical tasks related to patient care.
