Position OverviewWe’re looking for an experienced and passionate APRN to join our Connecticut and Massachusetts teams. You’ll provide bedside wound care in skilled nursing facilities, lead care coordination, and help shape best practices in our growing organization.
What You’ll Do- Conduct bedside wound assessments and provide hands‑on treatment
- Create and manage individualized wound care plans
- Collaborate with SNF staff, physicians, and families
- Document thoroughly
Educate patients, caregivers, and facility staff - Help build a new standard for wound care rooted in SNF expertise
- APRN with wound care experience
- Wound certification, or willingness to obtain it and be reimbursed
- Strong clinical judgment and communication skills
- Comfortable working independently in SNFs and collaborating with facility teams
- Willingness to travel within MA
